New Patient Registration

We have an open list and welcome requests for registration from patients living in or moving to the practice area. Our boundary area map can be found here: Practice Boundary Map

In order to register you need to complete a registration form (GMS1), a practice lifestyle questionnaire for each member of the family (please note – children under 5 only need a GMS1) and the sharing consent form for each member of the family registering. You can collect all of these forms from the surgery. We will make a note on your records if you prefer to see a particular doctor or nurse.

Once we have received all your completed forms your lifestyle questionnaire will be reviewed by one of the Practice Nurses who may invite you to make an appointment.

Change of Address

If you change name, address or telephone number, please let our receptionists know by filling in a form.

Please note that if your name changes we will need you to bring in some evidence e.g. a marriage certificate.

If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
